About the project

EHR+ streamlines electronic health record workflows by delivering data summaries, seamless record updates, and multi-role collaboration for a smoother clinical experience.

Health records sit at a specific clinical bottleneck that most medical software makes worse rather than better. A physician meets a patient and needs the full picture immediately: history, medications, lab results, prior visits, current concerns. The traditional EHR scatters that data across screens, tabs, and formats that demand navigation time the clinical moment doesn't allow. In high-stress, time-sensitive encounters, that navigation overhead directly impacts treatment accuracy. EHR+ was designed to close that gap by turning complex, multi-source patient data into a structured, accessible view, and that focus earns the case its place in the electronic health records platform conversation.

The design work spanned the full EHR+ experience, from patient intake through the clinical encounter into follow-up scheduling and record updates. Every screen answered a specific question. Can a clinician gain a comprehensive understanding of the patient's health status, log new concerns, and make treatment decisions, all inside a single interface that respects the pace of a real appointment. That principle is what separates a working online electronic health record from the fragmented tools that slow clinical decision-making across the industry.

The project introduced a new chief complaint feature that lets doctors quickly log essential patient concerns at the start of an encounter, creating a structured intake process that ensures critical concerns get documented upfront. Physicians can schedule follow-up examinations and record detailed visit notes inside the same workflow. That combination gives the electronic health record healthcare app the specific clinical utility that separates a genuine workflow tool from a digital filing cabinet.

The design rests on a clear hypothesis: presenting personal health data through interactive visual tools increases patient understanding and improves shared decision-making. Clean typography carries dense medical data at clinical reading speed. Functional color distinguishes data types and priority levels without overwhelming the clinician. Task

Project objective

  • EHR+ – Health Records Platform project approached us with a focused challenge: to turn complex, multi-source patient data into a structured and accessible digital experience. Our goal was to design a personal health record interface that helps patients and caregivers review health history clearly-even in high-stress, time-sensitive situations.

    We hypothesized that presenting personal health data through interactive visual tools would increase patient understanding and improve shared decision-making in clinical settings.

Problem

Understanding user pain points

  • Our discovery phase revealed several critical issues limiting the effectiveness of the current PHR systems. In many cases, doctors admitted it's faster to print records than interpret them digitally.

    Chronology is frequently lost due to scattered documentation, especially during hospitalizations when data volume increases rapidly. Information gaps also occur before visits, such as missing details on recent medications taken at home.

    Lastly, there’s no simple way to gain a quick, clear overview of the patient’s condition.
