EHR+ – Health Records Platform project approached us with a focused challenge: to turn complex, multi-source patient data into a structured and accessible digital experience. Our goal was to design a personal health record interface that helps patients and caregivers review health history clearly-even in high-stress, time-sensitive situations.
We hypothesized that presenting personal health data through interactive visual tools would increase patient understanding and improve shared decision-making in clinical settings.

Problem
Understanding user pain points
Our discovery phase revealed several critical issues limiting the effectiveness of the current PHR systems. In many cases, doctors admitted it's faster to print records than interpret them digitally.
Chronology is frequently lost due to scattered documentation, especially during hospitalizations when data volume increases rapidly. Information gaps also occur before visits, such as missing details on recent medications taken at home.
Lastly, there’s no simple way to gain a quick, clear overview of the patient’s condition.

The original timeline displayed patient data in a fragmented, difficult-to-read format, making it hard for clinicians to quickly grasp critical details. This lack of clarity made it challenging to accurately trace the sequence of events, often slowing down diagnoses and disrupting continuity of care.

Capturing the Patient’s Primary Concern
One major challenge in EHR+ – Health Records Platform ui / ux was the difficulty in quickly gaining a full understanding of the patient’s health status. Existing tools limited clinicians’ ability to capture a comprehensive overview, which slowed decision-making and impacted treatment accuracy.
Introducing a new chief complaint feature has greatly enhanced clinicians’ ability to record essential patient concerns immediately, improving the overall clinical workflow.

Starting Care with Clinical Clarity
To address this issue, we introduced the ability for doctors to quickly log a new chief complaint at the start of a patient encounter. This allows for a more structured intake process and ensures critical concerns are documented upfront.
In addition, physicians can schedule follow-up examinations, record detailed visit summaries, and add comments to each diagnosis-ensuring that the patient’s record remains complete, contextual, and clinically useful.

Physicians pointed out a persistent challenge with the existing EHR system: the structure was too complex for efficient use during patient care.
Many admitted they preferred printing multiple documents and working with paper records, as navigating the digital interface was slow, fragmented, and often counterproductive in time-sensitive scenarios.

To resolve this issue, we introduced a unified visit view where all relevant information is displayed in one place. The doctor can access the active visit note, view attached documents, see previous comments, appointments, and confirmed diagnoses-without switching contexts.
This approach improves clarity, reduces time spent navigating, and helps doctors focus on the patient, not the system.

All visual elements were defined within a detailed brand guide designed specifically for EHR use cases.
